There are three forms available. Modified LESFs have English and Filipino versions which are word document files. On the other hand, Modified Alternative Learning System (ALS) form is also downloadable which is a spreadsheet file. To download a copy, one just need to click the link of the form and it will be automatically saved in the device used in accessing the form.
In the modified form for incoming kinder to senior high school students, parents and guardians are asked if they are willing to allow their child to participate in limited face-to-face classes. There is also a question if what could be the sole reason why they are not for their children to attend in the said class setup.
